Claiming for indemnity
In order to speed up settling your claim, please print and send the notification loss forms and payment detalis forms together with the following documentation:
- ID document
- medical document issued by the medical service provider
- statement containing date, place and event occurrence condition
- in case of car accident police report
- in case of illness the Physician Statement Form has to be filled in by the treating / family doctor.
