Claiming for indemnity
In order to speed up settling your claim, please print and send the notification loss forms and payment detalis forms together with the following documentation:
- medical document issued by the medical service provider
- medical leave certificates
- receipts for treatment and medication costs
- in case of car accident police report
- in case of death: Death Certificate.
