All accident claims must be reported to Chartis.
To report a claim you must complete a claim form which is available in the section”Claims”.
The claim form must contain the policy number, medical reports and as specific information as possible. Additionally, your employer must sign the form.
In case of a claim regarding dental injury the dental report must be completed by a dentist.
